The Coordinated Veteran Care (CVC) program supports eligible DVA Gold and White Card holders with chronic conditions, including mental health concerns. The program connects you with a dedicated Patient Coordinator to help you manage your health and reduce preventable hospital visits.

Assistance with bookings through a personal Patient Coordinator.

Monthly check-in calls for ongoing support.

Home visits when required.

Coordination with your GP for urgent scripts, referrals, and health needs.
Our CVC team includes your treating GP, a Patient Coordinator, and experienced practice nurses, working together to keep your health on track.

Gaynor Williams, our CVC Nurse Coordinator, brings a wealth of experience and personal understanding to veteran care. As an Army wife of over 30 years, Gaynor is passionate about supporting veterans and their families through tailored healthcare planning and support.
