The Coordinated Veteran Care (CVC) program supports eligible DVA Gold and White Card holders with chronic conditions, including mental health concerns. The program connects you with a dedicated Patient Coordinator to help you manage your health and reduce preventable hospital visits.

Coordination with your GP for urgent scripts, referrals, and health needs.
Our CVC team includes your treating GP, a Patient Coordinator, and experienced practice nurses, working together to keep your health on track.
